Job summary

Valley Health System in Southern Nevada seeks an experienced Registered Nurse to lead discharge planning and utilization management activities. You will work to maximize reimbursement while coordinating with patients, families and the care team to ensure safe transitions.

Requirements include graduation from an accredited nursing program, 3+ years of varied clinical experience, and NV RN licensure; 2 years in utilization review/management or case management preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Minimum three years experience in varied clinical settings.
• Two years experience in Utilization Review, Utilization Management or Case Management preferred.
• Knowledge of social and physical factors that affect functional status at discharge, and knowledge of community resources to meet post discharge clinical and social needs.
• Computer proficiency including word processing, spreadsheets, and data collection/management programs.

Responsibilities

• Establish a safe, individualized discharge plan.
• Provide proficient timely utilization management services to maximize reimbursement for patient visits.
• Collaborate proactively with patient, family and healthcare team to achieve goals.
