Job summary

Qualifications
- Associate degree in Nursing or equivalent.
- Active RN license.
- 2+ years of medical/hospital nursing experience preferred.
Responsibilities
- Actively participates in rounds to review patient status and discharge plans.
- Educates patients and families regarding illness impact and post-acute resources.
- Documents discharge planning, evaluations, and barriers to progression of care.
- Communicates with payors for post-acute care authorization as needed.
- Coordinate post-acute care services to ensure timely discharge.
