Job summary

Qualifications

Responsibilities

• Identifies patients who would benefit from Case Management interventions based on initial discharge needs assessment.
• Assesses data by interviewing patient/family and evaluating needs.
• Develops and implements Plan of Care with the nursing team to improve patient outcomes and coordinate discharge planning.
