Qualifications

• Current RN licensure in state of employment, required.
• 2-year Associate Degree required; BSN or higher preferred.
• Weekend and holiday coverage required.
• 2+ years healthcare experience preferred; 1+ year case management or related experience preferred.

Responsibilities

• Lead complex discharge planning in collaboration with physicians and the care team.
• Assess, plan, implement and evaluate proactive/discharge plans.
• Coordinate care and act as a resource for discharge planning and post-acute options.
• Maintain knowledge of community resources for medically complex cases.
