Job summary

Advocate HealthCare is seeking a Registered Nurse in Care Coordination to manage comprehensive assessments, develop care plans, coordinate discharge planning, and collaborate with social work and multidisciplinary teams. The role supports patient transitions and utilization management within a 40-hour week schedule in North Carolina.

Ideal candidates will hold an RN license with 3+ years of nursing experience, and preferably a BSN.

Qualifications

• RN license required and eligible to work in North Carolina.
• 3 years nursing experience with clinical exposure.
• Case management or discharge planning experience preferred.
• Knowledge of CMS or regulatory guidelines preferred.
• Strong critical thinking and communication skills.
• Ability to work with multidisciplinary teams.

Responsibilities

• Perform comprehensive assessments and develop care plans with timelines.
• Coordinate discharge planning and patient/family communication.
• Provide case management across healthcare, finances, housing.
• Initiate referrals and track utilization reviews.
• Collaborate with social work and care teams to remove discharge barriers.
• Educate staff on admission status and regulatory requirements.
