Job summary

Mission Hospital in Asheville, North Carolina, is seeking a Case Manager to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans reflecting each patient’s needs, collaborating with physicians, nurses, and leaders to ensure safe transitions and high-quality care.

You will identify at-risk patients, perform comprehensive assessments, and drive discharge planning with the multidisciplinary team.

Qualifications

• 2+ years in case management or 3+ years in a clinical setting
• RN licensed in the state of practice
• BLS certification required
• InterQual experience preferred

Responsibilities

• Provide case management services for inpatient and observation patients as assigned
• Identify patients at risk for adverse outcomes during transitions
• Assess psychosocial, medical and discharge needs of patients/family
• Coordinate care plan and drive discharge planning with the multidisciplinary team
• Partner with Social Services to ensure post-acute needs and level of care
• Evaluate progression of care using InterQual criteria
• Act as liaison between physicians, patient/family, hospital staff and outside agencies
