Responsibilities

• Identify patients who would benefit from Case Management interventions based on initial discharge needs.
• Assess data by interviewing patient/family and evaluating needs.
• Develop and implement a Plan of Care with nursing to achieve outcomes.
• Collaborate with multidisciplinary team and providers to execute the Plan of Care.
• Monitor and adjust the Plan of Care; document actions in EMR.
