Job summary

TriStar Hendersonville Medical Center is seeking a registered nurse Case Manager to coordinate care across inpatient and observation services. You will promote patient-centered care, manage length of stay, and develop safe discharge plans in collaboration with the multidisciplinary team.

The role emphasizes utilization management, resource optimization, and effective communication with physicians, Social Services, and patients to ensure quality outcomes and timely transitions of care.

Qualifications

• RN license required
• Associate degree in nursing or nursing diploma required
• Bachelor's degree in nursing preferred
• 2+ years' case management experience or 3+ years clinical nursing experience required
• InterQual experience preferred

Responsibilities

• Provide case management for inpatient and observation patients.
• Identify patients at risk during transitions of care.
• Assess psychosocial, medical, and discharge needs and plan resources.
• Reassess clinical status and plan mitigations for readmission risk.
• Coordinate care plan and discharge planning with the multidisciplinary team.
• Refer to Social Services when psychosocial risks are identified.
• Engage patient and family in setting goals and expectations.
• Document recommendations and care coordination actions clearly.
