UNC Health in Hendersonville, NC, is seeking a Part-time Care Manager - RN to provide ongoing assessment, planning, implementation and evaluation of patient needs. The role aims to improve care continuity and cost effectiveness within care management, UR, and discharge planning.

The ideal candidate is an RN licensed in NC or a compact state, with 3 years of acute care experience and BLS certification; BSN preferred.

Qualifications

• Must be licensed to practice as a Registered Nurse in the state of North Carolina or one of compact states.
• Three years of recent clinical experience in an acute care setting/home health/long-term care.
• Basic Life Support (BLS) certification.

Responsabilités

• Reviews new admissions and prioritizes patients; documents plan of care in the EMR.
• Discusses discharge barriers with the team and coordinates family meetings when needed.
• Provides education on resources and coordinates referrals to community providers.
• Coordinates with units to update progression and adjust discharge planning.
• Collaborates with care team to solve issues for high-risk cases and follow up.
• Monitors admissions and stays; interfaces with departments and payers for approvals.
• Performs admission and continued stay reviews per UR plan and notifies patients accordingly.
