Key qualifications include 2+ years in case management or 3+ years in a clinical setting, active RN license in North Carolina, and BLS certification.

Qualifications

• 2+ years experience in Case Management OR 3+ years experience in a clinical setting.
• InterQual experience preferred.
• Current RN license in the state of practice.
• BLS certification required.

Responsabilités

• Provide case management services for inpatient and observation patients.
• Identify patients at risk for adverse outcomes during care transitions.
• Conduct comprehensive psychosocial, medical, and discharge assessments.
• Coordinate plan of care and discharge planning with the multidisciplinary team.
• Collaborate with Social Services to ensure appropriate post-acute care.
• Evaluate care progression using InterQual and document care management activities.
