Advocate Health's Case Management team in Charlotte, NC seeks a part-time RN Case Manager to perform assessments, plan care, and coordinate services for discharge planning. You will work weekends (Sat/Sun 7:00am-7:30pm) with a focus on patient-centered care and regulatory compliance.

You should have an RN license, 3 years of nursing experience, and case management background. BSN preferred; familiarity with Medicare/Managed Care helps ensure timely, efficient transitions and optimal patient

Formación

• Graduate of a Board of Nursing approved nursing education program (RN).
• Preferred BSN; current RN license or temporary license as RN.
• Typically requires 3 years of related nursing experience.
• Clinical experience within the assigned population.
• Case Management experience or background preferred.
• Strong critical thinking and analytical skills preferred.
• Professional certification preferred within 3 years of hire.

Responsabilidades

• Conducts complete assessments, establishes appropriate plans, and initiates interventions within desired timeframes.
• Collaborates and negotiates with patient, family, and team for discharge planning/transitioning care.
• Provides continuity of care and discharge planning services compliant with regulatory standards.
• Performs and documents utilization reviews per department standards.
• Collaborates with management, physicians, and care teams to ensure efficient progression of care.
• Maintains CMS regulatory awareness and tools for payer communications.
• Develops relationships with community resources to meet patient needs.
