Job summary

The role requires an active RN license in Florida, at least 1 year in hospice or hospital care, EMR experience, a valid driver’s license, and reliable transportation.

Qualifications

• Current license as an RN in the state where the employee will be working.
• Minimum of one year nursing experience in hospice or hospital setting preferred.
• Experience with an EMR/EHR system.
• Mobile driver with valid license and auto insurance per policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English (verbal and written).
• Ability to visit participants in their homes for assessments.
• Ability to perform essential functions including lifting and physical tasks; accommodations as needed.
• Active BLS for healthcare professionals from AHA/Red Cross.

Responsibilities

• Provides and manages direct care to patients and families as part of the IDT.
• Coordinates the Plan of Care reflecting patient needs.
• Educates patients and families about disease, prevention, palliative care, and safety practices.
• Reports changes in patient condition to IDT and other health professionals.
• Participates in IDT to evaluate level of care and referrals.
• Delivers concise oral and written reports to IDT; communicates with physicians and staff.
