Job summary

Chapters Health in Florida is seeking a compassionate RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care in home settings.

The role requires current Florida RN license, at least 1 year hospice or hospital nursing, EMR/EHR experience, valid driver''s license, and strong communication in English.

Qualifications

• Current RN license in Florida.
• Minimum of one (1) year nursing experience in hospice or hospital.
• Alzheimer''s Disease and Related Dementias training certification for Florida PACE locations.
• Experience with EMR/EHR systems.
• Valid driver''s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to use equipment with visual and auditory mechanisms.
• Ability to communicate in English (verbal and written).
• Ability to visit participant in their homes for assessments.

Responsibilities

• Provides and manages direct care to patients and families as part of Interdisciplinary Team (IDT).
• Coordinates the Plan of Care reflecting the patient''s evolving needs.
• Initiates skilled nursing interventions to enhance prevention, alleviate symptoms and maximize comfort.
• Documents patient/family contact information in the EMR and communicates with IDT.
• Completes initial and semi-annual assessment for all Company services and explains services to patients/families.
• Initiates Plan of Care with Physicians and IDT members to address immediate needs.
• Acts as Company representative at assigned facilities and facilitates referrals to all service lines.
• Communicates frequently with other members of the IDT and provides timely clinical communication using SBAR.
