Job summary

Chapters Health System is seeking an RN Case Manager to perform home-based nursing assessments and coordinate a comprehensive Plan of Care with the Interdisciplinary Team. You will focus on clinical, palliative, and supportive care to help patients remain in their home environments when possible.

The role requires an active RN license, 1 year of nursing experience (hospice or hospital preferred), EMR/EHR experience, and the ability to travel to patient homes.

Qualifications

• Current RN license in the state of employment (or eligible)
• Minimum 1 year nursing experience; hospice or hospital preferred
• Experience using EMR/EHR systems; valid driver's license and reliable transportation

Responsibilities

• Provide direct nursing care to patients and families as part of the IDT
• Coordinate Plan of Care reflecting patient needs and evolving status
• Educate patients, families, and caregivers about disease process, palliative options, and safety practices
• Communicate changes to IDT and obtain physician orders as needed
• Document thoroughly in EMR and maintain SBAR communications
• Represent the company at assigned facilities and coordinate referrals across service lines
• Support after-hours coverage and participate in Quality Improvement activities
