Job summary

Chapters Health is seeking an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the IDT, and provide clinical, palliative, and supportive care to keep patients in their home environment whenever possible.

The role requires an active RN license in Florida, BLS certification, and strong communication skills for home visits with families and physicians. A valid driver's license and reliable transportation are required for community visits.

Qualifications

• Current RN license in the state of employment (FL).
• Minimum 1 year nursing experience; hospice preferred.
• Alzheimer's/related dementias training required or to be obtained.
• Experience with EMR/EHR systems.
• Valid driver's license and auto insurance; reliable transportation.

Responsibilities

• Provide direct patient/family care as part of IDT.
• Coordinate Plan of Care reflecting evolving needs.
• Educate patients/families about disease, palliative care, safety.
• Home visits to assess safety, meds, nutrition, and DME.
• Report changes to IDT; participate in referrals/admissions.
• Maintain documentation per policy; SBAR communications.
