Job summary

Parkridge Medical Center in Chattanooga, TN is seeking a Case Manager RN to coordinate patient care from admission through discharge. You will lead the plan of care, assess needs, and collaborate with the multidisciplinary team to ensure safe transitions and appropriate resource utilization.

The role requires an RN license, an associate degree or nursing diploma, and 2+ years in case management or 3+ years in clinical nursing. Bachelor’s degree is preferred, and InterQual experience is a plus.

Qualifications

• RN License is required.
• Associate Degree in Nursing or Nursing Diploma is required.
• Bachelor’s Degree in Nursing is preferred.
• 2+ years’ experience in case management OR 3+ years’ experience in clinical nursing is required.
• InterQual experience is preferred.

Responsibilities

• Provides case management services for inpatient and observation patients as assigned.
• Identifies patients at risk for adverse outcomes during transition of care.
• Performs a comprehensive psychosocial, medical and discharge needs assessment.
• Reassesses clinical condition and develops mitigation strategies.
• Coordinates the plan of care and drives discharge planning with the care team.
• Partners with Social Services to ensure appropriate post-acute care.
• Refer patients to third party payer and disease/case management programs.
• Document care coordination interventions and discharge plans.
