Job summary

Mission Hospital in Asheville, North Carolina, is seeking a Case Manager to support patients and families through care transitions and coordinate services. You will collaborate with physicians, nurses, and social services to develop care plans, facilitate safe transitions, and ensure high-quality discharge planning.

This role emphasizes partnership with clinical teams and community resources to reflect each patient’s unique needs and to promote optimal outcomes.

Qualifications

• 2+ years experience in Case Management OR 3+ years in a clinical setting.
• InterQual experience preferred.
• Associates Degree in Nursing required; MSN preferred.

Responsibilities

• Provide case management for inpatient and observation patients.
• Identify patients at risk for adverse outcomes during care transitions.
• Assess psychosocial, medical, and discharge needs of patients and families.
• Coordinate care plans and discharge planning with the care team.
• Collaborate with Social Services to ensure post-acute needs and appropriate level of care.
• Monitor progression of care using InterQual criteria and escalate issues as needed.
• Act as liaison between physicians, patients, families, hospital staff, and outside agencies.
