Qualifications

• Graduate of an accredited school of nursing.
• 2 years of acute hospital case management or healthcare experience.
• Active RN license to practice in the state of practice.
• Bachelor's or Master's in Nursing preferred.
• ACM certification preferred.

Responsibilities

• Facilitate care along a continuum through effective resource coordination to help patients access appropriate care.
• Assess transition needs to promote timely throughput, safe discharge and prevent readmissions.
• Balance clinical and financial requirements and advocate for patient needs with prudent resource management.
• Promote prudent utilization of fiscal, human, environmental, equipment and services.
• Develop and implement a transition plan using patient assessment, choice and available resources.
• Identify patients at risk for readmission and refer to Social Work or Complex Case Review as needed.
• Screen patients for factors affecting progression of care and intervene to promote throughput.
• Perform related duties as required.
