Job summary

HCA Healthcare Trident Hospital in Charleston, SC is seeking a Registered Nurse (RN) Case Manager to support patients and families through the care journey. You will coordinate services, develop care plans, and ensure safe transitions in collaboration with physicians, nurses, and department leaders.

The role emphasizes coordinating plan of care, managing length of stay, and identifying at-risk patients during transitions to maintain quality care across the continuum.

Qualifications

• Graduate from an accredited school of Nursing (ADN or higher).
• Active RN license in state of practice.
• 1–3 years hospital nursing experience required.
• 2+ years in Case Management preferred.
• Case Management or Utilization Review certification preferred.

Responsibilities

• Coordinate plan of care and discharge planning.
• Identify at-risk patients during care transitions.
• Assess psychosocial, medical and discharge needs.
• Ensure appropriate post-acute care in partnership with Social Services.
• Evaluate progression using InterQual criteria.
• Refer patients to third-party payer and disease management programs.
• Advocate for patient rights and effective communication.
