Parkridge Medical Center in Chattanooga, TN, is seeking a Case Manager RN to coordinate patient care, plan discharges, and drive transitions with a multidisciplinary team. You will assess psychosocial and medical needs, ensure resource utilization, and promote safe, efficient care pathways.

The role emphasizes collaboration with Social Services, adherence to criteria (InterQual), and timely referrals to support post-acute care and patient outcomes.

Qualifications

• RN License is required.
• An Associate Degree in Nursing or Nursing Diploma is required.
• Bachelor’s Degree in Nursing is preferred.
• 2+ years of case management experience or 3+ years of clinical nursing experience are required.
• InterQual experience is preferred.

Responsibilities

• Provides case management services for inpatient and observation patients as assigned.
• Identifies patients at risk for adverse outcomes during transitions of care.
• Performs psychosocial, medical and discharge needs assessments.
• Reassesses clinical condition and readmission risk, developing mitigation strategies.
• Coordinates plan of care and drives discharge planning with the multidisciplinary team.
• Partners with Social Services to ensure post-acute needs and appropriate level of care.
• Ensures timely referral to Social Services when psychosocial risk factors are identified.
• Involves patient and family in identifying needs and setting goals.
• Evaluates progression of care using evidence-based tools and criteria.
• Makes referrals to third party payer and disease/case management programs.
• Facilitates patient throughput and safe transitions, focusing on quality and efficiency.
• Documents recommendations and care coordination activities for the health care team.
