TriStar Centennial Medical Center in Nashville, TN is seeking a Case Manager RN to coordinate services, develop individualized care plans and support patients and families through their care journey. You will work with physicians, nurses and leaders to ensure safe transitions and high-quality communication.

The role includes discharge planning, patient education and collaboration with the Rehab Program Director to deliver patient-centered care across the continuum.

Qualifications

• RN licensure in good standing and relevant nursing education.
• Experience coordinating patient care and discharge planning.
• Ability to collaborate with physicians, nurses and care teams.

Responsibilities

• Support patients and families through care journey and planning.
• Coordinate services, connect resources and develop care plans.
• Partner with providers to ensure safe transitions and clear communication.
• Document psychosocial assessments and discharge planning findings.
• Collaborate with Rehab Program Director and interdisciplinary team.
