Job summary

TriStar Summit Medical Center is seeking a Case Manager RN to coordinate patient care, manage length of stay and develop safe discharge plans in collaboration with a multidisciplinary team. The role focuses on quality outcomes, resource utilization and appropriate transitions of care from inpatient to next levels of care.

The ideal candidate will hold an RN license with ADN/ diploma and have 2+ years of case management or 3+ years nursing experience, preferably with InterQual experience.

Qualifications

• RN license required.
• ADN or nursing diploma required.
• BSN preferred.
• 2+ years’ case management or 3+ years’ clinical nursing experience required.

Responsibilities

• Provides case management services for inpatient and observation patients.
• Identify patients at risk for adverse outcomes during transitions of care.
• Assess psychosocial, medical and discharge needs and available resources.
• Reassess patient’s condition and plan -> educate on resources and prevent readmissions.
• Coordinate care plan and discharge with multidisciplinary team and physician.
• Collaborate with Social Services for post-acute needs.
• Refer patients to third party payer and disease management programs.
• Facilitate patient throughput with focus on quality and efficient transitions.

Education

Associate Degree in Nursing or Nursing Diploma
