Qualifications

• RN with graduate degree from an accredited School of Nursing.
• Must have excellent interpersonal communication and organizational skills.
• Ability to work with acutely and chronically ill patients and families.

Responsibilities

• Assess and document clinical, psychosocial and financial needs of patients and prepare care plans.
• Apply InterQual to determine level of service and coordinate with physicians and bed control.
• Identify plan of care, estimated length of stay and discharge plan with patients and families.
• Monitor treatment plan and ensure steps for discharge are completed and no gaps in care.
• Discharge/transition patients timely and document variances.
• Escalate concerns/barriers to appropriate treatment or transition.
• Maintain knowledge of facilities and resources available to patients and caregivers.
