Job summary

The Staff Pad is partnering with a respected healthcare organization in Hamilton, TX to recruit an experienced Case Management Manager who leads a high-performing team to optimize patient outcomes and safe transitions of care.

This RN-led role focuses on discharge planning, utilization management, regulatory compliance, and coordinating post-hospital care while supporting patients and families through the care continuum.

Qualifications

• Graduate of an accredited School of Nursing.
• RN license in Texas; CPR, BLS required; ACLS preferred.
• 5+ years RN experience; Case Management leadership experience preferred.
• Knowledge of utilization management, discharge planning, and regulatory requirements.

Responsibilities

• Conduct discharge assessments on admission and develop timely discharge plans.
• Perform daily patient assessments, monitor progress, and document changes.
• Coordinate post-hospital care and discharge to home or other settings.
• Educate patients and families; liaise among patients, families, physicians and care team.
• Document discharge services accurately per hospital policy.
• Maintain individualized care plans and update with status changes.
• Review denials and ensure appropriate appeal completion.
• Support access to financial assistance and community resources.
• Coordinate care and rehabilitation using social work expertise.
• Partner with Quality Improvement to enhance patient safety and monthly reporting.
