Job summary

The Registered Nurse (RN) Case Manager at Mayo Clinic in Arizona coordinates care across the continuum within an interdisciplinary team to optimize utilization and outcomes, while upholding patient advocacy and evidence-based practice.

This full-time role requires 4x10 hour shifts (0700–1730) with days; TN sponsorship is available and an RN license with BLS is preferred, along with CCM/ACM-RN certification where possible.

Qualifications

• Arizona BSN from ACEN/CCNE/NLN CNEA accredited program or in candidacy.
• If graduation not within 2 years, at least 1 year RN experience in applicable care setting.
• One year RN Case Management experience or completion of MCSHS RN Case Manager Fellowship within 6 months of hire.
• 3 years acute nursing preferred; 1 year Case Management preferred.
• Current RN license and BLS competency; state licensure or certs as required.

Responsibilities

• Coordinate patient care plans and utilization management across the continuum.
• Provide leadership and education on case management concepts within the team.
• Bridge communication between providers, patients, families and outside agencies.
