The position drives utilization management, care facilitation, and discharge planning, balancing patient needs with efficient resource use while supporting families and the care team.

Qualifications

• Graduate of an accredited nursing program; BSN preferred, or MSW from an accredited program.
• Current Texas RN license or LMSW license in Texas; LCSW preferred.
• Case Management certification within 2 years of hire.
• Three years nursing or social work experience in acute hospital-based care or similar setting.

Responsibilities

• Coordinates care progression across the continuum and communicates with care teams.
• Collaborates with physicians, nursing, and multidisciplinary staff to ensure timely care.
• Addresses barriers impeding diagnostic or treatment progress.
• Proactively identifies discharge delays and facilitates timely discharge.
• Seeks consultations to expedite care and coordinate services with payors.
• Documents in TQ screens and patient records; supports cost-effective care.
