Qualifications

• Graduate of an accredited school of professional nursing or MSW program.
• Active Texas RN or LMSW license; LMSW required, LCSW preferred.
• Certification in Case Management within two years of hire.
• Three years hospital-based nursing or social work experience.
• Strong interpersonal communication and negotiation skills.
• Demonstrated leadership and analytical abilities.

Responsibilities

• Coordinate care progression across the continuum with physicians and multidisciplinary teams.
• Maintain active communication with care team to ensure timely, appropriate patient care.
• Identify and resolve delays and barriers to discharge and care progression.
• Collaborate with payors and care teams to facilitate covered days and documentation.
• Lead and participate in clinical pathways and performance improvement initiatives.
• Ensure accurate documentation and reporting of outcomes and resource use.
