Responsibilities

• Coordinate patient care progression across the continuum.
• Collaborate with physicians, nursing and care team to ensure timely, appropriate care.
• Identify and resolve discharge delays and obstacles.
• Document care plans, LOS and resource utilization accurately.
• Lead/participate in performance improvement and clinical pathways.
• Act as mentor to new hires and support orientation activities.
