Job summary

Memorial Hermann Health System seeks a Manager of Case Management to support the Director in implementing the inpatient case management program, including care facilitation, utilization management, discharge planning and performance improvement. Qualifications

• RN licensed in Texas or LMSW required; LCSW preferred; Case Manager Certification required.
• Minimum five years in utilization management, case management, discharge planning or related cost/quality management.
• Three years of hospital-based nursing or social work experience.
• Three years of demonstrated leadership experience.
• Knowledge of managed care, inpatient, outpatient and home health care components.
• Excellent oral and written communication and collaboration with physicians.

Responsibilities

• Assist in supervising and managing all aspects of the local level program.
• Coordinate day‑to‑day operations of the program at the unit level.
• Lead performance improvement efforts and data-driven decision making.
• Partner with care teams to ensure safe, efficient care within budgetary guidelines.
• Support hiring, coaching and performance evaluations for staff.
