Job summary

Methodist Hospital is seeking an RN Case Manager to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans reflecting each patient’s needs, in collaboration with physicians, nurses, and leaders to ensure safe transitions and consistent quality across care settings.

Qualifications

• RN license required.
• Education: Associate Degree, RN Diploma, or BSN.

Responsibilities

• Guide patients and families through orientation, explain benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers for treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting the patient’s strengths, needs and recovery goals.
• Complete psychosocial assessments and discharge planning evaluations and document findings for care teams.
• Collaborate with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team for patient-centered care.
• Promote values of compassion, respect, and excellence in every interaction.
