Job summary

Memorial Hermann Health System in Houston is seeking a Case Manager to support physicians, primary medical homes and interdisciplinary teams, coordinating care, utilization management and discharge planning to improve outcomes and control costs.

The role requires RN or MSW background, Texas license, and strong communication, leadership and analytical skills; experience in care management and performance improvement is preferred.

Qualifications

• Graduate of an accredited school of professional nursing; Bachelors of Nursing preferred, or graduate of an accredited masters of social work program.
• Current and valid license to practice as a Registered Nurse in the state of Texas or Current and valid LMSW license in Texas; LCSW preferred
• Certification in Case Management within two years of hire

Responsibilities

• Coordinates patient care progression across the continuum.
• Maintains active communication with physicians and multi-disciplinary team to ensure timely, appropriate care.
• Facilitates discharge planning and utilization management to optimize LOS and cost.
