Qualifications

• RN license in Texas or LMSW in Texas required; LMSW preferred with LCSW optional.
• Certification in Case Management within two years of hire.
• Three years of nursing or social work experience in acute hospital-based setting or equivalent.
• Excellent interpersonal communication and negotiation skills; demonstrated leadership.

Responsibilities

• Coordinates and facilitates patient care across the continuum with physicians and multi-disciplinary teams.
• Monitors discharge planning, utilization management, and resource use to optimize patient flow.
• Collaborates with payors, and updates care plans and documentation in medical records.
• Leads performance improvement efforts and ensures continuity of care from pre- to post-acute stages.
• Fosters family engagement and supports individualized continuing care planning.
