Job summary

Ideal candidates will hold RN or LMSW credentials in Texas, have 3+ years of hospital-based nursing or social work experience, and be certified in Case Management within two years of hire.

Qualifications

• Graduate of an accredited school of professional nursing required; BSN preferred, or MSW program with Master’s degree preferred.
• Three years of hospital-based nursing or social work experience.
• Certification in Case Management within two years of hire.

Responsibilities

• Coordinates and facilitates patient care progression across the continuum.
• Collaborates with physicians, nursing and the multidisciplinary care team to ensure timely, appropriate care.
• Identifies delays and obstacles to discharge and resolves problems with payors as needed.
• Documents discharge planning and care management activities in patient records.
