The position may be full-time or part-time depending on need, with a focus on delivering quality, cost-conscious care and ensuring smooth transitions across the care continuum.

Qualifications

• Current, valid RN license (or OT/PT/SW as applicable) in the state of employment.
• BLS certification required.

Responsibilities

• Coordinate clinically complex patient care across the continuum and discharge planning.
• Ensure quality clinical and cost outcomes while addressing issues and arranging services.
