JourneyCare Hospice is seeking a compassionate Hospice RN Case Manager in Lake Forest, IL, serving Waukegan and Zion. You will manage a dedicated caseload, perform assessments, and plan care focused on comfort and dignity for patients and families.

Qualifications

• Graduate of an accredited RN program.
• Active Illinois RN license.
• Current CPR and First Aid certification.
• Hospice or home health experience preferred.
• Strong clinical judgment and communication skills.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed.
• Provide end-of-life support, including family education and pronouncement.
• Maintain timely, accurate documentation.
• Supervise Home Health Aides in compliance with regulations.

We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Lake Forest, Waukegan, and Zion areas.

Schedule & Territory
