Job summary

JourneyCare Hospice is hiring a Hospice RN Case Manager to join our growing team in Barrington, IL. Shift Information: Monday-Friday 8:30-5:00; On Call: one week every 12 weeks; Territory: Barrington, 60010.

You will manage patient and family caseload, coordinate with the interdisciplinary team, and ensure care plans are implemented and evaluated. Travel, documentation, and on-call duties are integral parts of the role.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid Certification.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance.

Responsibilities

• Assume primary responsibility for patient and family caseload through assessment, admission processing, planning, implementation and evaluation.
• Gather data from patient and family factors and coordinate with interdisciplinary team.
• Initiate communication with attending physician and agencies to coordinate care.
• Provide regular visits to reassess needs and update the plan of care.
• Perform clinical pronouncement of death and support family.
• Maintain accurate patient records and care plans.
• Supervise delivery of care by Home Health Aide as required by guidelines.

What You’ll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines

Qualifications:
