JourneyCare Hospice in Northbrook, IL is hiring a Hospice Registered Nurse (RN) Case Manager for a full-time role. The position offers manageable caseloads, interdisciplinary support, and a patient-centered approach to end-of-life care.

Daily schedule is Monday–Friday, 8:30 AM–5:00 PM with occasional weekend, summer, and winter rotations. Salary ranges from $80,000 to $97,000 plus a comprehensive benefits package.

Qualifications

• Graduate of an accredited RN program.
• Active Illinois RN license.
• Current CPR and First Aid certification.
• Hospice or home health experience preferred.
• Strong clinical judgment and communication skills.
• Valid driver’s license and proof of insurance.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed.
• Provide end-of-life support, including family education and pronouncement.
• Maintain timely, accurate documentation.
• Supervise Home Health Aides in compliance with regulations.

We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Northbrook area.

Schedule & Territory

• Territory: Northbrook, IL and surrounding communities.
• Monday–Friday, 8:30 AM – 5:00 PM. No On-Call
• One weekend rotation every 12 weeks and 1 summer and 1 winter holiday

Compensation & Benefits

• $80,000 – $97,000 annually (based on experience)
