Job summary

JourneyCare Hospice in Chicago, IL seeks a compassionate Registered Nurse to manage patient and family caseload within the hospice program. You will perform nursing assessments, coordinate with the interdisciplinary team, and ensure timely, quality care for patients in their homes.

Qualifications

• Must be licensed as a Registered Nurse in the state of practice.
• CPR and First Aid certification required.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Assume primary responsibility for patient and family caseload across nursing processes.
• Obtain data from patient physical, psychological, social, and spiritual factors affecting needs.
• Coordinate care by communicating with attending physician and interdisciplinary team.
• Provide regular visits to reassess needs and update the care plan.
• Perform clinical death pronouncement and support families.
• Maintain accurate patient records and timely care plans.
• Supervise and document delivery of home health care by aides as required.

Qualifications

Monday-Friday 8:30am-5pm and One Weekend Every 12th Week (4 weekends/year). 1 Winter Holiday and 1 Summer Holiday

What You’ll Do

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
