Addus HomeCare’s JourneyCare Hospice is seeking a compassionate Hospice RN Case Manager to deliver patient‑centered end‑of‑life care in Vernon Hills, IL. This full‑time role offers manageable caseloads and strong interdisciplinary support.

You will manage a dedicated hospice caseload, perform assessments and care planning, coordinate with physicians, and provide family education. Requirements include RN license, CPR/First Aid certification, and hospice or home health experience preferred.

• Strong clinical judgment and communication skills

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager
• Perform patient assessments, admissions, and individualized care planning
• Deliver skilled nursing care focused on comfort and dignity
• Coordinate care with physicians and interdisciplinary team members
• Conduct routine visits and adjust care plans as needed
• Provide end-of-life support, including family education and pronouncement
• Supervise Home Health Aides in compliance with regulations

We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Vernon Hills area.

Schedule & Territory

• Territory: Vernon Hills, 60061 and surrounding communities.
• Monday–Friday, 8:30 AM – 5:00 PM. No On-Call.
• $80,000 – $97,000 annually (based on experience)
• 401(k) with company match
