You will manage patient and family caseload, coordinate with the interdisciplinary team, and ensure high-quality end-of-life care across physical, emotional, and spiritual domains. Qualifications include RN licensure in the state, graduation from an accredited RN program, CPR/First Aid, quality assurance knowledge, and a valid driver’s license

Qualifications

• Graduate from an accredited RN program.
• Licensed as a registered nurse in the state of practice.
• CPR and First Aid certifications required.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance.

Responsibilities

• Manage patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation of the nursing process.
• Gather data from patient and family across physical, psychological, social and spiritual factors and coordinate with interdisciplinary team.
• Communicate with attending physician and team to coordinate optimal care.
• Provide regular visits to reassess needs and update care plans.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records and care plans; supervise home health aide delivery of care.

What You’ll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
