Job summary

Hartford Hospital is seeking a Case Coordinator to support patient throughput, assess admission needs, and coordinate care across the continuum. The role focuses on facilitating appropriate level of care and transitioning patients smoothly to the next level of care.

The successful candidate will coordinate with RNs, social work, and other team members, participate in utilization management, and contribute to quality initiatives within Hartford HealthCare.

Qualifications

• CT RN license
• ASN required; BSN preferred
• Two years acute-care and diversified nursing experience including medical/surgical nursing; case management, home care, or managed care experience considered
• Excellent communication, negotiation and conflict resolution skills
• Knowledge of computer applications and reimbursement systems
• Certification in case management desired

Responsibilities

• Assess patients for appropriate level of care and coordinate their care across the continuum
• Identify risk factors and facilitate involvement of support services
• Perform utilization management and develop transition planning activities
• Document care in case management applications and collaborate with health care team
• Contribute to process and quality improvement initiatives
