Job summary

Hartford Hospital is seeking a Case Coordinator to support patient throughput and coordination across the care continuum. You will assess patients for appropriate level of care, coordinate services across the care continuum, identify risk factors, and document aspects of care in our case management systems and related applications.

The role requires a CT RN license, ASN, and two years of acute-care experience, with BSN preferred.

Qualifications

• CT RN license is required for this role.
• ASN required, BSN preferred.
• Two years acute-care and diversified clinical nursing experience required.
• Excellent communication, negotiation and conflict resolution skills required.
• Knowledge of computer applications and reimbursement systems preferred.
• Certification in case management desired.

Responsibilities

• Assess patients for appropriate level of care.
• Coordinate care and services across the care continuum.
• Identify risk factors and facilitate involvement of support services.
• Develop and implement transition planning activities and document care in CM systems.
