Job summary

Hartford Hospital is seeking a Case Coordinator RN to uphold patient throughput by assessing need levels, coordinating care across the continuum, and guiding transition planning. The role emphasizes advocacy, utilization management, and collaboration with the care team to ensure timely, safe discharge planning for diverse inpatient populations.

Ideal candidates hold a CT RN license with ASN (BSN preferred) and at least two years of acute-care/nursing experience; case management or managed care

Qualifications

• CT RN license required.
• ASN required; BSN preferred.
• Two years acute-care, and diversified clinical nursing experience required.
• Case management, home care, and/or managed care experience strongly considered.
• Excellent communication, negotiation and conflict resolution skills required.
• Knowledge of reimbursement systems preferred.

Responsibilities

• Assess patients for appropriate level of care and coordinate their care across the continuum.
• Identify risk factors and facilitate involvement of support services.
• Develop and implement transition planning activities; document care in CM systems.
• Collaborate with health care team on process and quality improvement initiatives.
