Job summary

Qualifications

• CT RN license is required.
• ASN required; BSN preferred.
• Two years acute-care and diversified nursing experience required; case management, home care, or managed care experience considered.

Responsibilities

• Assess patients to determine appropriate level of care and care needs.
• Coordinate care across the care continuum and involve support services.
• Identify risk factors and implement transition planning and utilization management.
• Document care in case management systems and collaborate with the care team.
