Job summary

Hartford Hospital, part of Hartford HealthCare, is seeking a Case Coordinator RN to join our Case Coordination Department. The primary focus is facilitating patient throughput by assessing appropriate levels of care and coordinating services across the continuum.

You will identify risk factors, involve support services, and document care in our case management systems while collaborating with a multidisciplinary team to ensure timely transitions and high-quality patient outcomes.

Qualifications

• Must hold an RN license (CT).
• ASN required, BSN preferred.
• Two years acute-care and diversified clinical nursing experience; case management, home care, or managed care experience considered.
• Excellent communication, negotiation and conflict resolution skills.
• Knowledge of computer applications and reimbursement systems preferred.
• Certification in case management desired.

Responsibilities

• Assess patients for appropriate level of care and coordinate their care across the continuum.
• Identify risk factors and facilitate involvement of support services.
• Perform utilization management functions and develop transition planning activities.
• Document care in case management applications and collaborate with the health care team on quality initiatives.
