Job summary

Hartford Hospital is seeking a Case Coordinator to support patient throughput and discharge planning. You will assess patients for appropriate level of care, coordinate care across the continuum, and engage support services to ensure smooth transitions.

This role requires CT RN licensure and strong communication skills. The Case Coordination Department emphasizes collaboration, use of case management systems, and ongoing quality improvement to optimize patient outcomes in a high-volume teaching

Qualifications

• CT RN license required.
• ASN required; BSN preferred.
• Two years acute-care and diversified clinical nursing experience, including medical/surgical nursing; case management, home care, and/or managed care experience strongly considered.
• Excellent communication, negotiation and conflict resolution skills required.
• Knowledge of computer applications and reimbursement systems preferred.
• Certification in case management desired.

Responsibilities

• Assess patients for appropriate level of care; coordinate care and services across the care continuum.
• Identify risk factors; facilitate involvement of support services.
• Perform utilization management; develop and implement transition planning activities.
• Document care in case management computer applications; collaborate with health care team on process and quality improvement initiatives.
