Job summary

Required are an active CT RN license and at least two years of acute-care nursing experience, with case management or managed care experience strongly considered. Excellent communication and collaboration are essential in this role.

Qualifications

• Active CT RN license required.
• ASN degree required; BSN preferred.
• Two years acute-care and diversified clinical nursing experience; case management, home care, or managed care experience valued.

Responsibilities

• Assess patients for appropriate level of care and coordinate across the care continuum.
• Identify risk factors and involve support services as needed.
• Utilize case management documentation in electronic records and coordinate transitions of care.
• Collaborate with health care team on process and quality improvement initiatives.
