Job summary

Qualifications

• CT RN license required.
• ASN required; BSN preferred.
• Two years acute-care and diversified clinical nursing experience, including medical/surgical nursing.
• Excellent communication, negotiation and conflict resolution skills required.
• Knowledge of computer applications and reimbursement systems preferred.
• Certification in case management desired.

Responsibilities

• Facilitate patient throughput by assessing appropriate level of care and coordinating services across the care continuum.
• Identify risk factors and involve support services to ensure timely transitions.
• Document care in case management applications and collaborate with the care team on process and quality improvement.
