Job summary

Hartford Hospital is seeking a Case Coordinator to support patient throughput and discharge planning within the Case Coordination Department. The role focuses on assessing patients for appropriate levels of care and coordinating services across the care continuum to ensure smooth transitions.

You will work with a team of RNs and administrative staff to advocate for patients, document care activities, and contribute to process and quality improvement initiatives.

Qualifications

• Excellent communication, negotiation and conflict resolution skills required.
• Knowledge of computer applications and reimbursement systems preferred.

Responsibilities

• Assess patients to determine appropriate level of care.
