Job summary

Hartford Hospital is seeking a Case Coordinator (RN) to facilitate patient throughput and assess the appropriate level of care for inpatient populations. You will coordinate care and services across the continuum, identify risk factors, involve support services, perform utilization management, and document activities in our case-management systems.

As part of a collaborative nursing team, you will contribute to transition planning and quality improvement initiatives, ensuring patient advocacy

Qualifications

• ASN required, BSN preferred.
• Two years acute-care nursing experience; case management, home care, and/or managed care experience strongly considered.
• Excellent communication, negotiation and conflict resolution skills required.
• Knowledge of computer applications and reimbursement systems preferred.
• Certification in case management desired.

Responsibilities

• Assess patients for appropriate level of care and coordinate care across the continuum.
• Coordinate care and services across the care continuum; identify risk factors.
• Facilitate involvement of support services; perform utilization management functions.
• Develop and implement transition planning activities and document care.
• Collaborate with health care team on process and quality improvement initiatives.
